Government fee: check or money order. We sell money orders in the office.
Photos are optional. We can take them in the office, so wear dark clothing.
Lost passport replacement in Chicago is for people whose valid passport is lost or stolen. Reporting it cancels it, but does not replace it. To get a new one you apply in person on Form DS-11 and sign in front of an acceptance agent. Call us, then bring proof of citizenship and ID to our office by 3 pm. The hardest part is often proof of citizenship, because the lost passport was the proof.
Forms and documents for a lost passport
Forms:
- DS-64, Statement Regarding a Lost or Stolen Passport. You can file it online, or by mail with a photocopy of your photo ID. You can also report the loss on the DS-11 itself by giving details of where and when it was lost or stolen. If you skip those details, the State Department may pause your application and ask for a DS-64.
- DS-11, signed in person in front of an acceptance agent.
What to bring:
- Proof of U.S. citizenship, the original: usually a U.S. birth certificate or a naturalization certificate. A photocopy of the lost passport helps if you have one.
- A valid photo ID, such as a driver's license.
- What you know about the loss: the date, the place, and a police report if you made one.
- A passport photo. We take them at the office.
- A check or money order for the government fee, and payment for the $35 acceptance fee.

Child under 16: $186.87 or $160.00, plus $35. If you have no proof of citizenship, a file search is $150 more. Pay the State Department by check or money order. The $35 is a separate payment. Fees from the State Department fee chart, October 2026.
Who can apply
Anyone whose U.S. passport book or card is lost or stolen, inside the United States. If you are abroad, you apply at a U.S. embassy or consulate instead, and we cannot help from Chicago. If your passport is damaged rather than lost, the same DS-11 process applies.
A typical lost passport case
This is a typical situation, not a real customer. A traveller loses her wallet and passport on a train two weeks after booking a trip to London, six weeks out. She files a police report and orders a new birth certificate from the state, which arrives in a week. She calls us, then comes in before 3 pm with the birth certificate, her new driver's license and two payments. We take her photo and fill in the DS-11 with the loss details. She signs in front of an acceptance agent. Dates are estimates; the government sets processing times.

Lost passport questions
I found my passport after I reported it lost. Can I use it?
No. Once a passport is reported lost or stolen, it is no longer valid, even if you find it. Using it can get you delayed or refused entry to another country. Send the found passport to the State Department so it is cancelled. The fees for your replacement are not refunded.
Should I report it lost before I come in?
You can, online, and the State Department cancels the passport within one business day. Or you can report it on the DS-11 at your visit by giving the details of the loss. Either way works. Before you report it, check your bags, your car and your last hotel one more time. A report cannot be undone.
My birth certificate was in the same bag. What now?
Order a new copy from the vital records office in the state where you were born. Many states offer faster service for a fee. If you were naturalized, ask about a replacement certificate. If you cannot get any proof of citizenship, the State Department can do a file search for $150 with your DS-11 and a written request. Call us to plan the order.
My passport was stolen with my visa inside. Does the visa move to the new passport?
No. A visa is issued by the other country and stays in the passport it was printed in. When the passport is cancelled, plan to apply for the visa again with that country's embassy or consulate once you have the new passport. Ask them first; some countries have their own process for a lost visa.
I lost my child's passport. Is that different?
The child applies on DS-11 with both parents' consent, the same as any child under 16. One parent can use Form DS-3053 if the other cannot come. Add the details of the loss to the form. The fees are the child fees: $186.87 or $160.00 to the State Department, plus $35.
