Photos are optional. We can take them in the office, so wear dark clothing.
A first time passport in Chicago is for adults who have never had a U.S. passport. The same applies if your last passport was issued before you turned 16, or more than 15 years ago, because those cannot be renewed. It starts with Form DS-11 and your signature in front of a passport acceptance agent. You bring your papers to our office on Clark Street. We check them, fill in the form with you, and file it once it is signed.
Forms and documents for a first-time passport
Form: DS-11, Application for a U.S. Passport. You sign it in person in front of an acceptance agent at a nearby acceptance facility. Please do not sign it before then.
What to bring:
- Proof of U.S. citizenship, the original: a U.S. birth certificate, a naturalization certificate, or a previous U.S. passport.
- A valid photo ID, such as a driver's license or state ID.
- A passport photo. We take them at the office if you need one.
- If your name is different from the name on your birth certificate: the marriage certificate, divorce decree or court order that shows the change.
- A check or money order for the government fee, and payment for the $35 acceptance fee.

A typical first-time case
This is a typical situation, not a real customer. A 34-year-old has never left the country. A work trip to Toronto comes up five weeks out, and the company books flights. She calls in the morning and gets a same-day appointment. She brings her birth certificate, her driver's license and two checks. We take her photo, check her papers and fill in the DS-11 with her. She signs in front of the acceptance agent before 3 pm, and the application is filed. Dates are estimates; the government sets processing times.

First-time passport questions
My last passport was issued when I was 14. Is this a first-time case?
Yes, for the form. A passport issued before age 16 cannot be renewed, so you apply on Form DS-11 as if it were your first adult passport. Bring the old passport with you. It can help show your citizenship and identity. You still sign in person in front of an acceptance agent, and the $35 acceptance fee applies.
I can't find my birth certificate. What can I do?
Order a replacement from the vital records office in the state where you were born before your visit. If you have no proof of citizenship at all, the State Department can do a file search for $150. That needs Form DS-11 and a written request. Call us first so we can tell you if a file search fits your case.
Can I get a passport card at the same visit?
Yes. You can apply for a book and a card together on the same DS-11. The State Department fee for a first-time book and card is $160 instead of $130, plus the same $60 expedite fee. The card is not valid for international air travel. It works only by land and sea to Canada, Mexico, Bermuda and the Caribbean. Our add-on for the card is $79.
Why is there a separate $35 fee?
Every DS-11 is signed in front of an acceptance agent, and the acceptance facility charges $35 for that step. It is a separate payment from the State Department fee. It is not part of our fee and not part of the government fee shown in our price box. Plan for two payments on the day you sign.
My driver's license has my married name and my birth certificate has my maiden name. Is that a problem?
No, if you bring the document that links the two names. That is usually your marriage certificate. Bring the original, or a copy issued by the office that keeps the record. A plain photocopy you made yourself is not enough. If you changed your name more than once, bring a document for each change.
